*2026 Dress Giveaway Appointment Form

Attention High School Students in Grades 9–12! Come pick out a free prom dress and accessory with the Princess Project Sonoma Chapter!

Just fill out this form to reserve your appointment! (Please note: each teen may book only one appointment to get a dress with us. Multiple appointments under the same name/for the same person will be cancelled.)

Please bring proof that you’re in high school (A school ID, school email, progress report, report card, etc.) Each teen may select one free dress and one accessory.

All Teen Participants must have an individually completed liability waiver on file before being admitted to our dress giveaway boutique. After you book your appointment you will receive a link for the waiver in your confirmation e-mail. If you are a minor under the age of 18, a parent or legal guardian must sign the waiver.

If you’ve already booked an appointment and just need to reschedule to a different date, follow the instructions on the calendar to update your reservation.

If you are fully cancelling your appointment and do not plan to reschedule right now, please follow these steps:

Click the blue “Cancel Selection” button under your scheduled date and time to remove it from the calendar. (This step is important so you don’t end up with multiple appointments later.)
Select the bubble labeled “Yes, I am fully cancelling my appointment and will rebook at a later time” to confirm your cancellation. The appointment calendar should fully disappear. (You will not be able to submit your form cancellation until you do this!)
