Attention High School Students in Grades 9–12! Come pick out a free prom dress and accessory with the Princess Project San Francisco Chapter!

Just fill out this form to reserve your appointment!

Please bring proof that you’re in high school (A school ID, school email, progress report, report card, etc.) Each teen may select one free dress and one accessory.

All Teen Participants must have an individually completed liability waiver on file before being admitted to our dress giveaway boutique. After you book your appointment you will receive a link for the waiver in your confirmation e-mail. If you are a minor under the age of 18, a parent or legal guardian must sign the waiver.

2. Then, click on an available time button (for example, “10:00 AM”) to select your appointment.
3. If a time button cannot be selected it means that timeslot is fully booked. Please choose another available date and time!
